Young guy on anabolics came in with chest pain. Heart? Aortic dissection? CT scan showed a blood clot. VTE is a real risk. It can kill you!
Here's what matters: we know how to treat it. Anticoagulation works. Stable patients go home on a pill.
What most people don't realize: relative risk context matters. A woman on oral contraceptives has a baseline VTE risk of about 3–4x normal. Add smoking, and it climbs to ~10x. A man on anabolics? Real risk, but less than a woman who smokes on birth control—especially if his blood count (Hb/Hct) stays controlled.
The modifiable part: (don't pin ??!!) Polycythemia (high RBC count) from anabolics drives much of the thrombotic risk. If you're going to use, manage your hematocrit. And obviously, don't smoke.
