Text Messaging (SMS/MMS) Communications

Consent to receive texts is not a condition of receiving care or purchasing any membership.
Message frequency varies. Message and data rates may apply. You may opt out at any time by replying STOP, and reply HELP for help. We honor opt-out requests as required by law; opting out of texts will not affect your ability to receive care through other channels.
For established patients, clinical text communication is conducted through our secure, HIPAA-compliant electronic health record and patient portal. Standard mobile carrier text messages are not encrypted; please do not include sensitive health information in standard SMS.
We do not share your mobile number with third parties or affiliates for their own marketing purposes.
Third-Party Services
We may use third-party services to support Site operations, including website hosting and infrastructure, analytics (e.g., Google Analytics), appointment scheduling, payment processing, and email communication. These providers may receive limited personal information necessary to perform their services and are contractually required to protect it. We do not share protected health information with these providers unless they have signed a Business Associate Agreement as required by HIPAA.
Data Retention
We retain personal information collected through the Site for as long as necessary to fulfill the purposes described in this policy, or as required by law. Waiting list and inquiry information is retained until you request removal or the information is no longer needed.

Järn Haus MD PLLC ("Järn Haus MD," "we," "us," or "our") is required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice of our legal duties and privacy practices, and follow the terms of this Notice currently in effect. Right to Access
You have the right to inspect and obtain a copy of your PHI maintained in your medical record and billing records. To request access, submit a written request to us at the address below. We may charge a reasonable, cost-based fee for copies. We will respond to your request within 30 days (or 60 days if we notify you of a need for an extension).
Right to Request Amendment
You have the right to request that we amend your PHI if you believe it is incorrect or incomplete. To request an amendment, submit a written request to us at the address below, including the reason for the amendment. We may deny the request under certain circumstances and will provide a written explanation if we do so.
Right to an Accounting of Disclosures
You have the right to request a list of certain disclosures we have made of your PHI. This accounting does not include disclosures made for treatment, payment, or health care operations, or disclosures you authorized in writing. Because Järn Haus MD delivers care primarily through telehealth, the following additional considerations apply to the privacy and security of your information:

Harm Reduction Medical Care for Anabolic-Androgenic Steroid (AAS) and Performance-Enhancing Drug (PED) Use
PURPOSE OF THIS DOCUMENT
This document serves as both a disclaimer and an informed consent for patients receiving medical care related to the use of anabolic-androgenic steroids (AAS) and/or performance-enhancing drugs (PEDs). Please read this document carefully, ask any questions, and sign below before receiving care.
PHILOSOPHY OF CARE
This practice provides medical care using a harm reduction approach for patients who use, have used, or are considering the use of AAS and/or PEDs. The goal of this care is to reduce the medical risks and complications associated with AAS/PED use through education, monitoring, and evidence-based interventions where available.
This care is provided with the understanding that:
- Abstinence from AAS/PED use is the safest option and is always encouraged.
- Some patients may choose to continue using these substances despite medical advice to the contrary.
- Refusing to provide medical care to patients who will not discontinue AAS/PED use does not eliminate the risks — it increases them by driving patients away from medical oversight.
- All patients deserve access to competent, nonjudgmental medical care regardless of their personal health decisions.
IMPORTANT MEDICAL DISCLAIMERS
1. No Endorsement of AAS/PED Use
Nothing in this medical relationship should be interpreted as an endorsement, encouragement, or prescription of AAS or PED use for non-medical purposes. The use of AAS and PEDs for performance enhancement or cosmetic purposes carries significant and well-documented health risks, including but not limited to:
- Cardiovascular disease (heart attack, stroke, cardiomyopathy, arrhythmia, heart failure, venous thromboembolism)
- Liver damage (cholestasis, peliosis hepatis, hepatic tumors)
- Endocrine disruption (hypogonadism, infertility, hormonal imbalance)
- Polycythemia/erythrocytosis (increased risk of blood clots)
- Psychiatric effects (depression, anxiety, aggression, psychosis, suicidal ideation)
- Musculoskeletal injury (tendon rupture)
- Dermatologic effects (severe acne, hair loss)
- Kidney damage
- Death
2. Limitations of Medical Evidence
Patients should understand that:
- The medical evidence regarding harm reduction strategies for AAS/PED use is limited. Much of the available guidance is based on expert opinion, case series, retrospective studies, and extrapolation from related fields — not large randomized controlled trials.
- There are no FDA-approved protocols for "post-cycle therapy," on-cycle harm reduction, or management of AAS-related complications in the context of ongoing non-medical use.
- Any medications prescribed or recommended in this context (e.g., clomiphene citrate, human chorionic gonadotropin, aromatase inhibitors, statins) may be used off-label, meaning they are being used for purposes not specifically approved by the FDA.
- Monitoring and laboratory testing can detect some complications but cannot guarantee safety. Normal laboratory results do not mean that AAS/PED use is safe. Some serious complications (e.g., sudden cardiac death, arterial plaque rupture, hepatic tumors) may occur without prior warning signs on routine testing.
- Medical science does not yet fully understand all the long-term consequences of AAS/PED use, particularly with the wide variety of compounds, doses, and combinations used in practice.
3. No Guarantee of Outcomes
This practice makes no guarantees regarding:
- The prevention of any specific complication or adverse health outcome
- The success of post-cycle therapy or hormonal recovery after AAS cessation
- The preservation or restoration of fertility
- The reversal of any AAS/PED-related health effects
- The accuracy or completeness of harm reduction advice, given the evolving and limited nature of the evidence base
4. Legal Considerations
Patients should be aware that:
- The non-medical use of AAS is illegal under federal law (Anabolic Steroids Control Act) and may also violate state laws. This practice does not assist in obtaining, distributing, or manufacturing controlled substances for non-medical use.
- This practice does not prescribe AAS for non-medical purposes (e.g., bodybuilding, athletic performance, cosmetic enhancement).
- Medical records related to this care are subject to the same confidentiality protections as all other medical records under HIPAA and applicable state law. However, medical records may be subject to subpoena or court order in certain legal proceedings.
- If the patient is a competitive athlete, AAS/PED use may violate the rules of their sport or governing body. This practice does not provide guidance on evading drug testing.

Telehealth Policy

How Telehealth Works at Järn Haus MD
Järn Haus MD is a telehealth-first practice. The majority of our care is delivered remotely through secure, HIPAA-compliant video visits between you and a licensed physician. We hold telehealth to the same standard of care as an in-person visit — the only difference is the medium.
This page explains how our telehealth services work, what you need to participate, and what to expect.
What We Can Do via Telehealth
Most of the services Järn Haus MD provides can be delivered effectively through telehealth, including:

Initial consultations and medical evaluations

Hormone management and testosterone replacement therapy (TRT) evaluations

Anabolic harm reduction consultations

Laboratory review and interpretation
-
Prescription management, including electronic prescribing of medications

Follow-up visits and ongoing monitoring

Referrals and coordination with local providers, specialists, and laboratories

What Telehealth Cannot Do
Telehealth has limitations. Some conditions require hands-on examination, in-person procedures, or diagnostic imaging that cannot be performed remotely. If your provider determines that your condition cannot be safely or adequately evaluated via telehealth, they will let you know and help coordinate an appropriate in-person referral in your area.
Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency department immediately.
The following are examples of situations that may require in-person evaluation rather than a telehealth visit:

Acute chest pain, severe shortness of breath, or signs of cardiovascular emergency

Suspected deep vein thrombosis or pulmonary embolism

Injection site abscess or infection requiring incision and drainage

Severe psychiatric crisis or active suicidal ideation requiring immediate intervention

Any condition requiring hands-on physical examination, procedures, or imaging that cannot be obtained locally
If any of these apply to you, seek in-person care first. Your provider will clearly communicate when in-person evaluation is needed and will help coordinate appropriate referrals in your area.
What You Need for a Telehealth Visit
To participate in a visit, you will need:

A smartphone, tablet, laptop, or desktop computer with a working camera and microphone

A reliable internet connection
-
A current web browser or the telehealth application we direct you to use
-
A private, quiet location where you can speak freely without being overheard
You are responsible for making sure your device and internet connection are working before your scheduled appointment. If you run into technical issues during a visit, your provider will attempt to reconnect or reach you by phone. If the connection cannot be restored, the visit will be rescheduled at no additional charge.
State Licensing and Your Location
Telehealth law requires that your provider hold an active medical license in the state where you are physically located at the time of your visit — not where you live, but where you are sitting when the visit takes place.
At the beginning of each visit, we will ask you to confirm your name, date of birth, and current physical location. If you are not in a state where we hold an active license, the visit may need to be rescheduled.

Controlled Substance Prescribing via Telehealth
Järn Haus MD may prescribe controlled substances, including testosterone (a Schedule III controlled substance), via telehealth in accordance with federal and state law. All controlled substance prescribing via telehealth is subject to the following:

Prescriptions are issued only for a legitimate medical purpose by a licensed physician acting in the usual course of professional practice.

A synchronous, two-way audio-video visit is required. Audio-only (telephone) visits are not sufficient for controlled substance prescriptions under current federal regulations.

Your provider will check the applicable state Prescription Drug Monitoring Program (PDMP) as required by law.
-
Appropriate diagnostic evaluation, including laboratory testing, is required before initiation and during ongoing management.

Federal telehealth prescribing rules are currently governed by a temporary extension through December 31, 2026. Permanent regulations are expected before that date. We will communicate any changes that affect your care.
Privacy and Security
All telehealth visits are conducted over HIPAA-compliant, encrypted platforms. Your health information is protected by the same federal and state privacy laws that apply to in-person visits. We recommend participating in visits from a private location. If someone else is present in the room with you, please let your provider know at the beginning of the visit.
Informed Consent
Before your first telehealth visit, you will be asked to review and sign a Telehealth Informed Consent form. This form provides detailed information about the benefits, risks, and limitations of telehealth, your rights as a patient, and our clinical escalation procedures. Care will not begin until informed consent has been obtained.
